Description Expedited, focused assessment to determine fit for non-MSP covered services requested:
IV Lidocaine Infusion, Ketamine Therapies, Prolotherapy, BOTOX, Platelet Rich Plasma (PRP).		 Comprehensive functional and medical assessment with a multidisciplinary report addressing the biopsychosocial issues identified.
